To Schedule an Appointment, Please Follow These Steps

Step 1: Complete the Appointment Form

  • This allows us to match your contact information with your physician’s laboratory order.
  • Most laboratory orders do not include your phone number or email address. We require your contact information so we can schedule your appointment and communicate with you if needed.

Step 2: Send Your Lab Order

  • Have your healthcare provider fax your laboratory order, or email us a copy. We will match your lab order with the appointment information you submitted.

Step 3: Schedule Your Appointment

Once we receive your appointment request and laboratory order, we will contact you to schedule a convenient date and time for your mobile laboratory collection.

Step 4: We Come to You

  • A certified phlebotomist will visit your home, office, or care facility, collect your laboratory specimen, and deliver it to your selected laboratory.
